This is a general outline of several scenarios.
You decide which direction you want to go and we will determine how best
to achieve your goals.
CONSULTATION ONLY (Assumes you have a patient you are considering.) A. At your location:
This may include any or all of the following:
- Initial evaluation, demonstration casting, demonstration modification, check
socket fitting, one or more patients, including introduction of professional
and/or technical staff to the system (two to three days).
- No Prosthesis is fabricated at this time.
- For a certification-training course, an extra day or two may be required, depending
on system you are training in.
Fee: $2500.00 per day
(One half-day minimum)
(Fee includes my expenses. Travel time is billed at 30% of the daily consulting
fee, prorated by the hour, payable on my departure.)
(If the need is urgent, any extra cost for short-notice airfare will be invoiced
in addition to the fee.)
B. In Salt Lake City:
You come to our facility in Salt Lake City and using one of our demonstration
patients, this visit may include any or all of the following:
- Initial evaluation,
demonstration
casting, demonstration modification,
check socket fitting, (two to three days).
- For a certification-training course, an extra day or two may be required, depending
on system you are training in.
- Visit Motion Control Inc. (the manufacturer
of the Utah Artificial Arm) with possible tours of the Center for Engineering
Design, where the Utah Arm was developed, and/or SARCOS Research (Dr. Jacobsen’s
development company), depending on time and scheduling.
Fee: $1800.00 per day.
COMPLETE UPPER LIMB FITTING -- Start to finish: C. At Your Location:
Utah Arm or equal-- (Up to one week at your facility):
- Utah Arm expert w/ 26 years experience.
- Funding consultation, development and application also available.
- Acquisition of necessary Utah Arm components, initial evaluations, EMG
testing, casting, socket design, check socket fabrication and fitting,
harness design, fabrication and fitting, patient electronic adjustment
and training, etc.
- Your occupational therapist may also wish to participate in the training
aspects of the program. (For an additional fee, we will bring our occupational
therapist with us to provide training).
- A functional prototype prosthesis is generally the result of this fitting
period.
- Central fabrication of definitive socket also included as well as one-year
telephone support.
- A second follow-up visit may be scheduled as required within the first
year.
Certification as a *Utah Qualified Practitioner available.
Fee: 45% of retail cost of prosthesis
(Fee includes my expenses.)
(If the need is urgent, any extra cost for short-notice airfare will
be invoiced in addition to the fee.)
(We request 50% down payment prior to the fitting.) D. At Our Offices in Salt Lake City:
Utah Arm or equal-- (Up to one week at our facility in Salt Lake City):
You and your patient (and perhaps you OT) come to our facility in Salt
Lake City. Very comfortable accommodations are available a mere four
blocks from our office, which is fifteen minutes from the Salt Lake International
Airport, five minutes from the University of Utah Medical Center, twenty
minutes from Motion Control, Inc., Otto Bock's Seating manufacturing
facilities and Springlite.
Includes all services in C. above, including training
by our OT at no extra charge. If appropriate, definitive prosthesis fabrication
can also
be achieved here at the same time.
Includes:
- Utah Arm expert w/ 26 years experience.
- Funding consultation, development and application also available.
- Acquisition of necessary Utah Arm components, initial evaluations, EMG
testing, casting, socket design, check socket fabrication and fitting,
harness design, fabrication and fitting, patient electronic adjustment
and training, etc.

(We request 50% down payment prior to the fitting.)
*Utah Qualified Practitioner:
During the course of this fitting, your designated professional staff
member(s) (must be ABC Certified Prosthetist) can also be qualified as
an official Utah
Arm trained prosthetist if he or she can:
- Participate full time in the fitting process;
- Participate full time in the technical seminars presented on the Utah Arm technology
(this includes arm control & operation theory, disassembly and reassembly,
maintenance, fine-tuning to the patient, etc.)
- Take and pass all daily quizzes and the final exam.
A second patient, either a Utah Arm candidate or not (or a potential Utah arm
patient who is sitting on the fence!), may be fit simultaneously so as to provide
the additional practitioner(s) with the needed hands on experience to qualify
for Motion Control's Trained Practitioner Program.
ProControl or equal --(up to 3 days at your facility)
Includes:
- Qualified expert with 26 experience.
- Initial evaluations, EMG testing, casting, socket design, check socket fabrication
and fitting, preparation of preparatory prosthesis, patient electronic adjustment
and training, one-year telephone support.
- A functional prototype prosthesis is generally the result of this fitting period.
- Central fabrication of definitive socket also included as well as one-year
telephone support.
- Funding consultation, development and application also available.
Fee: 50% of cost of prosthesis
(Fee includes expenses -- see A. above)
HOW IT WORKS BEST
1. Decide on how you want me to help.
Feel free to use us to assist you in determining the Rx, component selection,
pre-authorization verbiage, etc.
2. Decide on the consulting option you desire, i.e.,
- Do you want just a consulting visit or a training course?
(This would entail a daily consulting fee.)
- Do you have a patient and want me to go to your office?
- Do you want to bring your patient to Salt Lake City?
3. Once the format is decided, a consulting agreement will be drawn up that
will detail the plan as you and I determine it.
This will include estimates for time involved, fees that apply, components
required and who is responsible to acquire them, etc.
4. Sign and return the consulting agreement via mail, fax or email, along with
your deposit of $500.00
- This deposit allows us to begin the scheduling process, including our staff
support and appointment schedule, airfare, your staff and appointment schedule,
scheduling your patient, working together on coding, determining and acquiring
needed tools, equipment and components.
5. You will need to send us:
- Patient data-- Email or fax me all pertinent information regarding the patient--
copies of patient evaluations, component and procedure plans, x-rays if appropriate,
photos and/or videos, etc. (Patient’s name may be removed from all documentation,
in order to preserve privacy in compliance with HIPPA, or we sign the appropriate
HIPPA form to allow these records to be viewed by us.)
- Cost estimates must be available for review so we can help with the authorization
effort. This is also how we begin to determine our fee.
I recommend that scheduling coincide with or follow definite written authorization
from the patient's insurance company or funding agency.
Scheduling:
All on-site fittings and training courses must be scheduled at least 30 to
60 days in advance to allow for local scheduling and travel arrangements. All
fees include normal expenses; however, short notice travel expenses, i.e.,
travel that must be scheduled in less than 21 days, will be added to the fee.
Travel time is billed at 30% of the daily fee. Payment Arrangements:
Accompanying the consulting agreement as designed together with you is agreement
fee of $500.00. This amount is credited against the final invoice.A down payment
of half of our fee is due prior the initial meeting with the patient. The
balance is due on completion of the initial fitting consultation.
Or, if we are fabricating the definitive socket, when this is delivered to
you. Socket fit cannot be guaranteed NOTE:
Most Utah Arm fittings, typically, can be divided into approximately equal
thirds based upon the cost of components. In this way, the wholesale cost of
the components can be accommodated and the balance of the fee shared to accommodate
the cost of training, fitting and fabrication as well as the need for future
follow up on the local level.
Our fee arrangement is designed to allow the local practitioner to become trained
during the course of his first Utah Arm fitting. However, if time or other
factors do not allow the practitioner to avail him or herself of this opportunity,
the fitting can still go on as if we were merely a part of the local staff.
The practitioner would not be qualified, in this case, to independently fit
any subsequent Utah Arm candidate or purchase components from Motion Control,
Inc. without taking an approved Utah Arm course.
